Scanning the webpages of http://www.lacity.org I came across the solution to this problem:

But these "valuable services to customers" are not about to deliver themselves. YOU have to request them. Dips, potholes, torn-up streets, or any of the other problems below, slow down traffic. So if you are aware of any of these, especially dips, potholes or torn-up streets anywhere in L.A., please fill out the request form or call the number below.
